I hereby grant to the AF Soccer Training, a division and registered tradename of Finy Enterprises LLC, its legal representatives, successors, and assigns, and those acting with its authority and permission (jointly and severally, “AF Soccer Training”) the right and permission to copyright, use pictorial depictions of and/or voice recordings of such child or adult, alone or in conjunction with others, made through any media or process, whether now known or unknown, without restriction as to changes, alternations, or reproductions for distribution, sale, broadcast, exhibition, publication, transmission or any other purpose whatsoever, whether commercial or educational. I grant to AF Soccer Training, its legal representatives, successors, and assigns, and those acting with its authority and permission (jointly and severally, “AF Soccer Training”) the right and permission to copyright, use pictorial depictions of, and/or voice recordings of such child or adult, alone or in conjunction with others, made by or on behalf of AF Soccer Training from AF Soccer Training rehearsals, performances, or scheduled events or otherwise, through any media or process, whether now known or unknown, without restriction as to changes, alternations, or reproductions for distribution, sale, broadcast, exhibition, publication, whether commercial or educational. I waive any right to inspect or approve such depictions or recordings, the copy or printed or electronic matter that may be used in connection with them, or the use to which they may be applied. And I hereby release and discharge AF Soccer Training, its representative, coaches, trainers and training personnel, staff and trainees and their parents and guardians, and/or Alex Finy from any liability, claim, or anything whatsoever; by virtue of, without limitation, any blurring, distortion, alteration, optical illusion, or use in composite form that may occur or be produced in the taking, processing, distribution, broadcast, exhibition or publication, or transmission of the depiction or recording or anything whatsoever related to or concerning; Injury resulting in any way from participation in any training, program, travel related to playing or training in any way whatsoever and of any type whatsoever to the person or well-being whether physical or mental; or mental health of the “player” or “trainee” including but not limited to any injury arising out negligence or an intentional act of a fellow trainee or otherwise of any type whatsoever, whether known or unknown, present or future or past or patent or latent. I certify, represent, and warrant with the full knowledge and acknowledgment of AF Soccer Training’s reliance upon the same, that I am the parent or guardian of the player/trainee identified above, and that I have the authority to make the above grant, waiver, and release on behalf of such child or adult in accordance with the provisions above; That I have the power to bind my spouse, the child’s other parent, or my fellow guardian or person of similar authority; That I knowingly accept responsibility for the failure to fill out any blank, failure to disclose any medical or mental or psychological condition or limitation of the trainee/player, or any ambiguity in this agreement or anything stated herein; and I will indemnify and hold harmless the AF Soccer Training and those I have released herein above including specifically Alex Finy; 4.1) from any claim to the contrary to the preceding paragraphs (1) or (2) immediately preceding or the grants, wavier, or release in the initial paragraphs of this document including paragraphs 1 through 4.2 in the preceding section of this document or in any way connected to any matter herein released or discussed or dealt with; 4.2) from any claims by or on behalf of my son, daughter or ward designated in the third line of this document or his or her estate as the case may be; and 4.3) all consequences and results of the breach of any warranty or representation herein, including but not limited to all litigation or investigation costs, attorney fees, and related costs.
